The Surgeon stands broad and heavy in a blood-dark apron, his body stitched together so many times it is hard to tell what was original. One ruined eye stares from a butchered face, while an extra grafted arm works beside him with a cleaver always close at hand. He looks less like a man than a workshop that learned to walk.

Plot Hooks

- The Surgeon is collecting specific body parts from the dead, and the party learns he has a list of living donors he plans to visit next.
- A local family begs for help after their missing son returns home alive, altered, and calling the Surgeon “father.”
- The Surgeon offers to save a dying ally, but demands a piece from each party member as payment.
